Deleted task reappearing

I have had a particular task that I created several years ago. Since then, I have had to DELETE this task several times; nevertheless, it shows up, but not with any particular time interval - sometimes, it will not show up for months, and then others, it will show up every day for a week. Each time, I manually delete the entry. Any ideas as to how to delete this entry once and for all, short of deleting all my tasks and restarting?
